How does PCOD affect fertility?

PCOS (Polycystic Ovarian Syndrome) or PCOD (Polycystic Ovarian Disease) are generally interchangeably used even though they have very minute differences. While PCOS affects the metabolism of the body as a whole PCOD affects only the ovaries. PCOD disrupts the functioning of the female reproductive system and affects the fertility of a woman. The major cause of such disruptions and disease is the imbalance of hormones occurring in a female body.

The condition should be curbed when it comes to the fore to avoid bigger complications later. This condition generally occurs in middle-aged females who become mature enough to bear children, but it might show itself in adolescence too. It is also genetic. There are certain ways of ascertaining PCOD. If these conditions or symptoms of a PCOD are visible the person should consult a good medical consultant immediately.

  • Irregular periods
  • Extreme hair fall
  • Abnormal hair growth
  • Acne
  • Skin darkening
  • Obesity

Irregular periods may be due to PCOS but PCOD is a disorder of abnormal release of sex hormones. The female reproductive system has parts like ovaries, fallopian tubes, uterus and vagina, each having specific functions to perform in the reproduction cycle. The ovaries hold the immature eggs. The functioning of the ovaries is determined by the pituitary gland. The communication between the two is through the production of hormones like the FSH (Follicle Stimulating Hormone) and Luteinizing Hormone pumped into the bloodstream. These hormones mature the eggs and the follicle size increases. Simultaneously estrogen is released to release Luteinizing Hormone in the ovaries. Then the ovulation occurs with the most mature follicle releasing the egg. This is the normal process of ovulation but if you have PCOD then the pituitary gland releases an abnormal amount of Luteinizing Hormone which in turn does not allow the follicles to mature and thus it cannot ovulate. These follicles generally dissolve after ovulation but in this care may not dissolve and may form a cyst causing infertility. Another condition is when the pituitary gland secretes an abnormal amount of insulin which in turn initiates the secretion of high levels of testosterone. This imbalance in secretion prevents the ovulation process causing infertility. High hormone imbalance also leads to other problems like abnormal hair growth and acne.

Other types of disorders that can be associated with PCOD like type 2 diabetes, heart disease, high blood pressure, cholesterol abnormalities, endometrial cancer and the irregular period leading to obesity.

PCOD can be diagnosed and treated with the right kind of medicinal help from an expert gynecologist. If you are looking for a specialist who could help you with your PCOD condition, the best place to go is Mohak infertility center with the best team of gynecologists and embryologists who can give the appropriate solution for your condition and even cure you of your infertility issues. To More Post: –Advantages and disadvantages of ICSI

Advantages and disadvantages of ICSI

Intracytoplasmic Sperm Injection (ICSI) is used for cases where there is extreme infertility of the male. This is also a form of IVF (in vitro fertilization). This is mostly used when several normal IVF procedures have failed. This is done by taking a single sperm and injecting it directly into the egg.

This procedure as we know now that it is done for extreme male infertility which may include conditions like:

  • Oligospermia where the sperm count of the male is extremely low so the chances of the sperm meeting the egg are negligible. This makes the fertilization process almost impossible.
  • Teratozoospermia where the sperm shape of the sperm is abnormal and is not good for fertilization.
  • Asthenozoospermia is another condition that may cause infertility due to the inability of the sperm to swim through the reproductive tract of the female and reach the egg to fertilize it.
  • If the eggs do not fertilize even after it is found that there is a good number of eggs and the sperm count also is found to be normal and healthy this method is used to insert the sperm into the egg.
  • ICSI is also done when the frozen sperm looks inactive.
  • In the case of In Vitro Maturation (IVM), where the egg is taken out of the ovaries just before maturation and the rest of the maturation is done in the lab, ICSI is done.
  • Preimplantation Genetic Diagnosis (PCD) also requires ICSI to decrease the interference of the sperm cell that is nearby the embryo and give accurate PCD results.
  • In cases where sperm is extracted through the TESE technique it is important to use ICSI.

The above cases show that while extreme Male infertility may be the reason to use ICSI there may be a few conditions other than Male infertility that may need ICSI.

Now we know why the technique is used have understood in layman terms how it is done that seems very simple but it is a highly advanced method. Even after being a highly advanced method, it has some disadvantages which we can also term as the risk factors or the cons of the technique. Due to this drawback, the technique is being argued among the medical fraternity for its use in regular conditions and regularly.

The proud feeling of having a baby with the male partner’s sperm is the main reason to go for this technique when the male partner is facing extreme infertility with conditions like low sperm count, low sperm motility or abnormal shape of sperm. Even male partners who have undergone vasectomy can also father a child using this technique, where the sperms are directly extracted from the testicle with the TESE technique and then injected into the egg.

Even though this technique is considered to have a very low chance of any risk, it does come with some risk factors like any other technique. Every technique has its pros and cons, nothing is perfect in this world. Once the sperms are obtained the male partner is free of any risk from the procedure and the technique is also risk-free or the risks are highly negligible. The main know risk factors of the ICSI technique are:

  • Embryo damage during the ICSI technique.
  • Couples using ICSI and IVF together may face multiple pregnancies or conceiving more than one baby which may cause complications to the mother during pregnancy and also while giving birth.
  • In the ICSI technique, there is also a very low but risk of major birth defects in comparison to normal pregnancy.

The above risk factors are arguably the factors that lead to discouraging ICSI with every IVF cycle. It is argued that it should be used only in understandable medical conditions where it is invariably the only treatment.

To More Post: –Uterine fibroid

Uterine fibroid

Noncancerous growths in the uterus during the childbearing year of a female called leiomyomas or myomas are generically called Uterine fibroid. They are not associated with any type of uterine cancer and they may never develop into cancerous tissue.

Most of the women have uterine fibroids at some time during their lives but as it does not cause any visible symptoms it goes undetected. These fibroids range in size from an undetectable seed to masses that can distort and enlarge the uterus. The uterus could be enlarged where it could sometimes touch the thoracic cage and also add weight to the body. Due to the mostly asymptomatic nature of the Uterine fibroid, it is most natural that this cannot be felt by the female and a doctor may also stumble on the fibroid in a pelvic examination or a prenatal ultrasound.

Research and clinical experience point towards a few causes that may be the reason for the Uterine Fibroid, lets check on them to increase our awareness about Uterine Fibroids.

Genetic changes or changes in genes that differ from the normal uterine muscle cells may cause Uterine fibroids to form in the Uterus

Hormones like estrogen and progesterone which stimulate the development of the lining of the uterine have been studies to promote the growth of Uterine fibroids. Studies also show that fibroid cells have more estrogen and progesterone receptors than the normal cells of Uterine muscles. These fibroids tend to shrink after menopause which points towards these two hormones in promoting the growth or development of Uterine fibroids. The fact that they shrink after menopause is also attributed to the decrease in the flow of estrogen and progesterone once a female attains menopause.

Another factor that promotes the growth of fibroids that came up during studies is the ECM (Extracellular Matrix). This material that keeps the cells together is more in Fibroids and they also contain growth factors pointing towards the generation of fibroids caused by the change enhanced by ECM in cells.

Fibroids vary in shape, size and growth patterns. There may be no similarity in them. Some fibroids grow slowly, some may grow very fast and some may remain the same size after their growth. Fibroids also tend to have growth spurts and some of them shrink after some time. Fibroids present during the pregnancy period of a female could shrink or disappear after the pregnancy when the uterus goes back to its normal size.

Mostly the fibroids don’t hinder a woman from getting pregnant as most of them are very small and dormant. But there have been cases where fibroids have caused infertility or pregnancy loss, especially when the fibroid is submucosal. Fibroids are also known to raise the risk of certain pregnancy-related complications like fetal growth restriction, placental abruption and preterm delivery.

Mohak Infertility Center, the top IVF center in Indore takes care of all the pre and post-pregnancy complications and does the necessary rectification to ensure a normal and complication-free delivery. To More Post: –Tips to cope with Infertility Stress

Tips to cope with Infertility Stress

Infertility a hurdle in your path towards parenthood can be quite devastating in terms of emotional balance and for relationships. Every couple dreams of a happy family with offspring of their own. Nature sometimes puts us and our relationships to the test with a very sensitive topic as infertility. In India, it is every more sensitive and tabooed. Every family member would be looking up to a new couple to quickly become a family. Everyone loves children around. Just in case you are facing problems in conceiving or bearing a child beyond the expected time of the family members they put a lot of pressure on the couple. Family pressure is not the only factor at work, even the partner in a relationship puts pressure on each other, peer group and you take on a lot of unwanted pressure of the situation. This could prove very damaging to your self-esteem, the relationship with your spouse and could lead to many other psychological and physiological conditions.

De-stressing through your infertility period or the period you are being treated for infertility could be quite testing but it has to be done to keep you in a balanced state of mind and away from psychosomatic disorders.

Let’s look into some very vital and effective methods of de-stressing during these highly disturbing times you go through while facing infertility problems and deciding on how to plan your family.

  • Understanding what you are feeling about the infertility problem and the condition is perfectly normal. Going through the tests and procedures could drain you emotionally, physically and financially. The feeling of uncertainty about the outcome or loss of control of your body could be very disturbing.
  • Share your fears, feelings and questions with your close kin or to a counselor if available in the facility you are getting treated for infertility. Sharing your fears and feeling can always ease a lot of burden on your mind.
  • Crying and being angry when you get bogged down by the surrounding pressure and negativity can surely ease you out a lot. Keeping your feelings locked up inside you could do a lot more harm than being able to vent it in any form to make you feel relieved.
  • Journal jotting is a very comforting and soothing activity when it comes to de-stressing. Journals are friends where you can pour out your emotions and avoid judgemental or unwanted responses from the other person.
  • Staying connected is one of the best remedies for any pressure or stress or a depressing state. Having good company and joyful people around you could be most of the time very relaxing and can take your mind off disturbing topics.
  • Infertility may take a toll on you and your spouse as both will be going through emotional turmoil and family pressure with unspoken resentment and feeling of inadequacy. Talking and discussing or even going to a counselor about your feeling and resentments could take a lot of load off the couple. It is always important to back your spouse in such a matter and be supportive throughout the process.
  • Strained strings between the couple could be rekindled with a bit more tenderness and romance brought back into your life to get your moods up and the love and support between the spouses. This could surely pep up the spirits and positive mindset in the couple to fight the situation together.
  • One of the worst causes of stress is uncertainty and ignorance about the situation and your future. The more you are aware of the situation, the process towards normalcy and the amount of time you would take to get back on track would release a lot of stress arising out of anxiety and uncertainty. Being aware of your chances of being a parent, the options available to your condition of infertility, the financial aspect and your current medical condition could make the future more clear and make your clogged-up mind a bit more eased.
  • Trying or adopting one of the different techniques to reduce stress like picking up a hobby, meditating, support group therapy and exercising could be of great help.
  • Another excellent way of calming yourself is by practicing deep breathing techniques which can be done anywhere or in any situation alone or with your spouse. This technique almost always calms you down in anxious, tensed or stressed situations.

Infertility can surely stress you out but you need to learn to cope up with stress and keep yourself calm and poised at all times and think positive.

Fallopian tube and its role in the reproductive system

Fallopian Tube, the name comes from anatomist Gabriele Falloppio who also named many other anatomical structures. The Fallopian Tube is also known as the uterine tubes, salpinges or oviducts. In layman’s terms, these are 10-12 cm long tubes that stretch from the uterus to the ovaries and are a part of the female reproductive system. They are not just a part but they play a very important role in the reproductive system. Any birth defect, injury, condition or disease can cause the reproductive system to fail and could tend couples childless.

There are two ovaries attached to the J-shaped Fallopian tubes which in turn are attached to either side of the uterus. They are the form the major part of the reproductive system of a woman. A Fallopian tube is anatomically described to have four major parts which have different roles in the reproductive system.

Fimbriae finger-like ciliated projections that help to collect the ovum or egg from the surface of the ovary when it is ready for ovulation.

The Infundibulum is a funnel-shaped opening near the ovary to which the fimbriae is attached which is the opening of the Fallopian tube through which the eggs enter.

Ampulla the widest section of the uterine or Fallopian tube where the actual conception or fertilization mostly happens.

Isthmus a narrow section that directs the fertilized eggs towards the uterine cavity and is the ending of the Fallopian tube.

An oviduct or Fallopian tube is lined by ciliated epithelium which helps n the movement of the ovum towards the uterus. The major function of the Fallopian tube is to transport the oocytes after ovulation towards the uterus. Before the oocytes reach the uterus the eggs meet the sperm in the ampulla region of the Fallopian tube. If the fertilization takes place the Fallopian tube will move the zygote with the help o the ciliated epithelium and muscle movement of the Fallopian tube towards the uterus for further growth of the zygote. If in case the egg is not fertilized it get degenerated here.

The Fallopian tube is the place where the initial development of the embryo takes place. The embryo enters the uterine cavity only after five days and implants itself in the uterus for further growth.

There may also be cases where the embryo gets implanted in the Fallopian Tube itself which is termed as a tubal pregnancy or ectopic pregnancy. This happens when there is any damage to the lining of the Fallopian tube that moves the egg to the uterus and is unable to move the egg and the egg gets implanted in the Fallopian tube itself. This could cause internal bleeding and rupture.

The Fallopian tubes are a very major part of the reproductive system and they are also can play a very major role in family planning. A Fallopian tube could be manipulated to stop the egg from going towards the uterus and thus avoiding pregnancy. Tubectomy is one such procedure where the Fallopian is blocked to prevent the transport of the ovum to the uterus for sterilization in females.

The fallopian tube also plays a major role in alternative methods that are used to get pregnant like the IVF technique of ZIFT (Zygote Intrafallopian transfer) and GIFT (Gamete Intrafallopian Transfer ) in which the zygote or the gamete is transferred to the Fallopian tube for the further process of pregnancy.

Fallopian Tube disorders and treatment.

Fallopian tubes are two tubes that connect the uterus and the ovaries providing a pathway for the eggs and the sperms to unite. This pathway where the egg moves from the ovaries where it gets fertilized by a sperm and moves on to the uterus to be implanted on its wall for further growth. It is the place where the first process of pregnancy starts from. If there is any major damage to the pathway would lead to problems in pregnancy and may even stop you from being a parent.

The fallopian tube is a very initial and very crucial part of fertility and pregnancy, it is essential to know about the various forms of disorders, ailments, or conditions it may have which might hinder the proper process of pregnancy. Damages of blocked Fallopian tube are major causes of infertility along with other conditions that may be detrimental to pregnancy.

The fallopian tube may get damaged due to infection and the formation of scar tissues. Pelvic inflammatory disease (PID), results from bacterial infection of the reproductive organs of a female. PID may also ascend from the vagina and cervix on to other parts of the reproductive tract caused by diseases like gonorrhea and chlamydia. PID may be a result of other conditions like salpingitis and pyosalpinx which can also damage the Fallopian tube.

Other conditions, diseases and disorders that may affect the proper functioning of a Fallopian tube are listed below.

  • Surgery in the pelvic or lower abdomen
  • Rupture of appendix
  • Damage caused by Intrauterine device(IUD) to the opening of a Fallopian tube
  • Ectopic pregnancy where embryo implanted in the Fallopian tube
  • Endometriosis
  • Uterine fibroid
  • Benign Fallopian tube tumors
  • Fallopian tube carcinoma
  • Abnormal Fallopian tube by birth.

All these conditions and diseases can be very damaging both to the Fallopian tube and the process of pregnancy itself rendering the couples children less. • Hysterosalpingography (HSG) checks for structural damages or problems in the Fallopian tube through imaging. This imaging is done after the woman’s menstrual period ends.
  • Sonohysterography is another imaging technique that can detect problems in the Fallopian tube. This image is taken through the ultrasound method to reveal abnormalities.
  • Hysteroscopy is a direct viewing technique where a thin viewing tube with a light is used to probe inside the Fallopian tubes to check for growths blocking the Fallopian tube. This insertion can also be used to remove polyp or fibroid seen blocking the Fallopian tube by minimally invasive surgery.
  • Laparoscopy is also another tubular device with a lighted camera through a small incision in the uterus to check for blockages in the Fallopian tube and can also remove the block by inserting apt instruments through laparoscopy itself.
  • Surgery may be the last option or the only option in some cases where other options have failed to provide results. In cases like ectopic pregnancy or infection, surgery is required to repair the damage done. In some cases even after surgery for repair of the Fallopian tube the woman may need to use IVF to conceive. To More Post: – Causes and symptoms of Blocked Fallopian Tubes

Causes and symptoms of Blocked Fallopian Tubes

The part of the female reproductive system that connects the ovaries with the uterus is called the Fallopian Tubes. During ovulation Fallopian tubes carry egg from an ovary to the uterus. Conception of the egg by the sperm occurs in the Fallopian Tube. If a conception occurs in the Fallopian tube the egg is carried through the Fallopian tube to the uterus for implantation on the uterus wall where it will grow to be a fetus.

A blocked Fallopian Tube will hinder the egg being carried in it for fertilization and the through passage to the uterus, after conception.

Causes of Blocked Fallopian Tube:

Scar tissue or pelvic adhesion blocks Fallopian Tubes. These blocks may be caused by many factors. Let us look into some of the major factors that cause Fallopian tube blocks:

  • Endometriosis, a condition where endometrial tissue builds up to block the Fallopian tube. Endometrial tissue formation on the outside of other organs could cause adhesion which may become a cause for Fallopian tube blockage.
  • Hydrosalpinx or scarring cause pelvic inflammatory disease which leads to blocked Fallopian tube.
  • STI (Sexually Transmitted Infection) like Chalmydia and gonorrhea can cause scarring and pelvic inflammatory disease.
  • Fallopian tubes can also be scarred by past ectopic pregnancy.
  • Fibroid growth in Fallopian tube in the area where the Fallopian tube attaches itself to the uterus can also cause blockage to the passage of the eggs.
  • Abdominal surgery done in the past especially on the Fallopian tubes themselves could lead to pelvic adhesion and become a cause for the blockage of the tubes.

Many of the above causes, pivotal to blocked Fallopian tubes, cannot be prevented. The only way to ensure a safe and through passage of the eggs is by getting it tested when you are not able to be pregnant and get the best opinion and option for this blockage. One such institution where you can get an authentic and trusted advice and opinion is Mohak Infertility Center. Symptoms often are not very obvious with blocked Fallopian tubes. Some of the women may never know that their Fallopian tubes are blocked. The only time when they may realize that they have blocked Fallopian tube is when they try to get pregnant and they face problems in conception. Some women may feel mild and regular pain on one side of the abdomen. This pain is usually caused due to a blockage named hydrosalpinx. In this condition the Fallopian tube is filled with fluid and blocks the Fallopian tube. Some conditions like endometriosis which lead to blockage of Fallopian tube could be very painful and can cause heavy periods and pelvic pain.. It increases the risk for blocked Fallopian tubes.

All You Need to Know About Ovulation

Ovulation

The process of releasing eggs from the ovaries to the Fallopian tube is called Ovulation. The cycle of ovulation will differ from woman to woman and also from mensuration cycle to cycle. The process of ovulation happens between 13th – 15th day before the next menstrual period. The egg has to be fertilized by the sperm in the next 12-24 hours, before the egg degrades. The sperm can live in the uterus for about 3-5 days, during which if ovulation happens it may fertilize an egg. After fertilization the egg attaches itself to the lining of the uterus and will develop into a fetus.

Ovulation cycle is a very important process in the woman’s body, not only as a part of the process of giving birth to a child, it is also a process in the body which initiates a lot of hormone flow necessary for the well being of a women.

Ovulation is one of the four stages of menstrual cycle.

  • Mensuration: This stage is where the uterus eliminates its thick lining formed for the attachment of the egg to be developed as a fetus. It releases this layer if no fertilization has taken place as the menstrual fluid  which contains blood, cells from lining and mucus.
  • Follicular Phase: Follicular Phase starts with the first day of mensuration and ends when ovulation starts. This is the Phase a hormone called the FSH (follicle stimulating hormone) is released. This stimulates the ovary to form follicles around the immature egg and transport it to the Fallopian tube. As soon as the follicles develop the uterus thickens in anticipation of a fertilized egg to attach itself to the uterus.
  • Ovulation: Ovulation start when the follicle releases the matured egg into the fallopian tube and this egg will remain alive for another 12 to 24 hours ready to be fertilized and it will die if no fertilization takes place in that period. This release is prompted by the high levels Luteinizing Hormone. During these two phases a lot of hormones as directed by the brain is released to help a follicle to prepare an egg and make it ready for fertilization. According to the needs of the egg hormones like oestrogen, follicle stimulating hormone, gonadotrophin releasing hormones and Luteinizing hormone.
  • Luteal Phase: This is the phase where the progesteron and oestrogen is released by the ruptured follicle which has taken the role of corpus luteum to release the two hormones that help the lining of uterus to stay thickened in anticipation of a fertilized egg to attach to it.

Knowing the ovulation period is very crucial for a woman who is intending to be pregnant. The knowledge about ovulation and ovulation cycle and its fertile period would a good factor in a successful conception. The knowledge of this period could also be good to prevent conception and help in family planning. The ovulation cycle is also a very critical biomarker for the good health of the woman. A regular menstrual cycle is a sign of good health and that all the hormones and glands secreting the required chemical are working as programmed and in good shape.

Any change in Ovulation cycle should be brought to the notice of a trained medical practitioner, particularly a gynecologist. This will help in diagnosing the underlying factors affecting this change and prevent a lot of health issues. What is Difference between IVF and test tube baby?

IS test tube baby born in a test tube.

Test tube baby the term that came into India, very popularly among the medical sector and the common public, for babies born other than natural conception.
